Nine health plans to provide MI Coordinated Health starting Jan. 1, 2026.

The Michigan Department of Health and Human Services (MDHHS) has awarded contracts to nine health plans to provide a new benefit plan, MI Coordinated Health, launching Jan. 1, 2026. 

MI Coordinated Health will be available to individuals enrolled in both Medicare and Medicaid in select counties in 2026 before expanding statewide in 2027. MI Coordinated Health is the successor to the MI Health Link program, which ends Dec. 31, 2025. Current MI Health Link enrollees will have the option of moving into MI Coordinated Health with no break in coverage. 

“MDHHS remains committed to an improved health care experience for low-income seniors and people with disabilities who are both Medicare and Medicaid enrollees,” said MDHHS Director Elizabeth Hertel. “MI Coordinated Health emphasizes comprehensive care coordination for all members, including home- and community-based services and nursing-facility care for qualified enrollees.”  

Nine health plans were selected to administer program benefits on the state’s behalf. The seven-year contracts include three, one-year optional extensions. Contracts were awarded based on the state’s 10 Prosperity Regions, with the Detroit Metro Prosperity Region awarded on an individual county basis. 

The following regions will be included in the Jan. 1, 2026 launch: 

  • Region 1 – Upper Peninsula Prosperity Alliance: Upper Peninsula Health Plan, LLC.  
  • Region 8 – Southwest Prosperity Region: Aetna Better Health of Michigan Inc.; Meridian Health Plan of Michigan, Inc.; Molina Healthcare of Michigan, Inc.; UnitedHealthcare Community Plan, Inc.  
  • Region 10 – Detroit Metro Prosperity Region – Macomb: Aetna Better Health of Michigan Inc.; HAP CareSource; Meridian Health Plan of Michigan, Inc.; UnitedHealthcare Community Plan, Inc. 
  • Region 10 – Detroit Metro Prosperity Region – Wayne: Aetna Better Health of Michigan Inc.; AmeriHealth Michigan, Inc.; HAP CareSource; Humana Medical Plan of Michigan, Inc.; Meridian Health Plan of Michigan, Inc.; Molina Healthcare of Michigan, Inc; Priority Health Choice, Inc.; UnitedHealthcare Community Plan, Inc. 

The following regions will be included in the Jan. 1, 2027 launch: 

  • Region 2 – Northwest Prosperity Region: Meridian Health Plan of Michigan, Inc.; Molina Healthcare of Michigan, Inc.; UnitedHealthcare Community Plan, Inc. 
  • Region 3 – Northeast Prosperity Region: Meridian Health Plan of Michigan, Inc.; Molina Healthcare of Michigan, Inc.; UnitedHealthcare Community Plan, Inc. 
  • Region 4 – West Michigan Prosperity Alliance: Aetna Better Health of Michigan Inc.; Meridian Health Plan of Michigan, Inc.; Molina Healthcare of Michigan, Inc.; Priority Health Choice, Inc.; UnitedHealthcare Community Plan, Inc. 
  • Region 5 – East Central Michigan Prosperity Region: Meridian Health Plan of Michigan, Inc.; Molina Healthcare of Michigan, Inc.; UnitedHealthcare Community Plan, Inc. 
  • Region 6 – East Michigan Prosperity Region: Aetna Better Health of Michigan Inc.; HAP CareSource; Meridian Health Plan of Michigan, Inc.; UnitedHealthcare Community Plan, Inc. 
  • Region 7 – South Central Prosperity Region: Aetna Better Health of Michigan Inc.; Meridian Health Plan of Michigan, Inc.; UnitedHealthcare Community Plan, Inc.

MI Coordinated Health is the result of information gathered from the MI Health Link program combined with a robust stakeholder feedback process going back to 2022, which included surveys and feedback from beneficiaries, advocates, providers and health plans.  

The resulting program is driven by five project pillars: 

  • Fostering integration and continuity. 
  • Reducing racial disparities. 
  • Improving care delivery. 
  • Promoting self-determination. 
  • Building a culture of quality.
